Fall Registration is now Open!! CVSA Now has Mail in Registration Available (Instructions Here). Save time, print your form and fill it out to bring to the in-person registration or use the mail in procedure below. MAIL IN REGISTRATION PROCEDURE
1. Please review the Frequently Asked Questions section on this website for details on CVSA program.

Many of the forms are in PDF format. Adobe Reader is required. A recent upgrade to correct a security flaw may cause your browser to automatically get the upgrade before you open a document. It is best to get the most current version by selecting the icon below. 3. Check the appropriate Age Group by dates provided Boy Groups (A, B & C) Girl Groups (D, E & F). 4. Provide Player’s Name, Address, Birth Date, Home Phone No., Mother’s & Father’s Name, a parent’s work no. (optional) and e-mail address. 5. Provide the last six digits of child’s social security number. This information is used for PA West ID#. 6. It is mandatory to complete the volunteer section of the registration. Failure to complete This section will VOID your child’s registration and your child will not play. (NO EXCEPTIONS)!
7. Any special requests made are not guaranteed.
8. Make checks payable to CVSA. Return registration form and for the appropriate amount to: CVSA, P.O. Box 151, Presto, PA 15142. You should be contacted by your child’s coach no later than Mid to end of March who will provide you with a practice/game schedule.
